About Appeals

If you received an order letter from the Division of Liquor Control, you may appeal this order with the Liquor Control Commission.

How to File an Appeal

To file an appeal with the Commission, a permit holder or legal representative must submit the following TWO documents:

1) Notice of Appeal (click here for the form); and

2) A copy of the Division’s Order being appealed

*To obtain a copy of the Division’s Order being appealed (including Tax Non-Renewal Orders), contact the Division of Liquor Control at (614) 644-2360 or email them at: fileinquiry@com.ohio.gov

Submit your appeal electronically (via email) or by post mail

Withdraw an Existing Appeal

To withdraw an existing appeal with us, please fill out the following form and submit to us via email or post mail.
